Services Provided
Foundation provides financial assistance, paid directly to the funeral home, to families providing us with proof of income. This income must meet the criteria of the United Sates Department of Health and Human Services Poverty Guideline Chart. Proof of eligibility means we will assist with the loved one’s funeral expense.
To be eligible you must fall into one of three categories:
- Deceased and party responsible for the funeral expenses receive assistance from Social Security, Social Security Insurance and/or Supplemental Security.
- Deceased and party responsible for funeral expense receives assistance from Welfare via Ohio Works First and Disability Assistance.
- Deceased and party responsible for funeral expenses receives earnings that fall within United States Department of Health and Human Services Federal Poverty Guidelines
